Spa Policies
By submitting your intake form and waivers you are agreeing to adhere to our posted spa policies. Please review them prior to your appointment by clicking the button below.
First Time Guests
As a first time guest you will be required to complete a thorough consultation form at least 48 hours prior to your scheduled appointment. This ensures that we have the proper information on file to prepare the best treatment for your needs.
Returning Guests
All returning guests must also complete an intake review form 48 hours prior to their return visits to ensure any updates to diet, lifestyle, stress and/or medications are properly noted so that modifications can be made for your safety and desired results.
COVID-19 Information & Consent
15 minutes prior to your appointment you must complete the COVID Information & Consent form or your appointment will be cancelled and charged per the cancellation policy posted on the Spa Policies Page.
